Our Responsibilities
We aim to:
- Treat you with respect, fairness, and courtesy.
- Uphold and protect your legal and human rights.
- Value your views, opinions, personal circumstances, and cultural beliefs.
- Deliver support that recognises and respects your preferences, choices, interests, and cultural and sexual identities.
- Communicate with you openly, honestly, and promptly.
- Safeguard your personal information, using it only with your consent.
- Review your services annually to ensure they continue to meet your needs.
- Consult with you about decisions regarding how services and supports are provided.
- Deliver high-quality, person-centered care and support always.
- Provide information and advice about other services and supports available to you.
- Give you opportunities to share feedback about our services.
- Protect your privacy and confidentiality in accordance with the Australian Privacy Principles and the South Australian Government Information Sharing Guidelines for Promoting Safety and Wellbeing.
- Ensure all staff are appropriately trained to meet your ongoing support requirements.
- Support your right to use an advocate to promote, protect, and ensure your full and equal enjoyment of your human rights.
Your Responsibilities
We kindly ask that you:
- Inform Community Support Inc about how you wish services and supports to be delivered to best meet your needs.
- Provide accurate and up-to-date information about your health and support needs, including any changes that occur during the service agreement period.
- Complete all relevant consent forms to ensure we can provide the best service to you; these forms will be reviewed and updated annually.
- Treat Community Support Inc staff, representatives, and volunteers with courtesy and respect.
- Understand that behaviours such as harassment, violence, abuse, aggression, theft, or property damage violate Community Support Inc’s values and will be addressed appropriately.
- Ensure a safe and risk-free environment for Community Support Inc staff while they work in your home, including maintaining a smoke-free environment during their presence.
- Avoid engaging in any illegal activity while staff are in attendance.
- Communicate any concerns about the services and supports being provided to you or your representative.
- Provide at least 24 hours’ notice if you need to cancel a scheduled appointment, service, or support.
- Provide at least 14 days’ notice if you wish to terminate your service agreement.
- Notify Community Support Inc if your NDIS plan is changed, suspended, or replaced with a new one.
- Inform Community Support Inc if you have used all your funding.
- Take responsibility for payment of any services provided by Community Support Inc that cannot be claimed from your NDIS plan
